How is the 5% Streamlined (SDOP) penalty actually calculated?

I think I fall under SDOP. People keep saying '5% penalty' but 5% of what, exactly? Is it 5% of every year's balance added together, or 5% of the highest balance I ever had? I want to estimate the cost before I commit.
