Publication 519 says I need a statement to establish my residency termination date. What kind of details belong in that file?

I understand the broad concept that an earlier residency termination date needs more than just my own conclusion, but I have not yet seen a practical checklist for the statement the IRS wants. Publication 519 says there is a signed statement requirement, which makes me think the supporting facts need to be organized more carefully than a normal travel note.

What I am trying to build is a file that is actually useful if someone has to look at it later. I want to know what categories of information belong there so the statement is tied to a coherent set of travel, identity, and foreign-connection facts rather than a vague summary.
